Primary healthcare services are an essential part of one’s overall well-being. Many people in need of medical attention lack health insurance and the means to purchase it – leaving them thinking they have nowhere to turn and with no healthcare home.
A medical home is not a building, house or hospital, but rather an approach to providing continuous, comprehensive primary care. Think of a medical home as a hub or home base where a patient’s medical history is known and their medical care is coordinated. Patients have an ongoing and personal relationship with their physician and a place to turn for their healthcare needs.
HANDS of St. Lucie County collaborates with partner physicians, hospitals and other healthcare providers to increase access to primary healthcare, to reduce unnecessary visits to local emergency rooms, and to provide a continuum of care to qualified, low-income, uninsured, adult residents of St. Lucie County.
