Mission

Our mission is to provide primary health care services to the indigent, medically uninsured and under insured people of St. Lucie County.  To accomplish this mission, HANDS seeks to:  

• Provide access to health care preventing serious and costly consequences of undiagnosed and untreated illness (e.g. Diabetes)
• Coordinate existing health services with patient navigation.
• Identify gaps in health care services and act as a medium to extend services to low-income and uninsured St. Lucie County citizens. 

We envision a system that provides access to primary health care services, referrals to specialists when needed, health education, and chronic disease management for those in need.
