About our Diabetes Education
Approximately 23.6 million people in the United States have Diabetes, half of which are undiagnosed.
Diabetes is one of the top deadliest diseases in the US. Complications of Diabetes include heart disease, stroke, vision loss, kidney disease and amputations. Diabetes is a chronic disease that has no cure. Diabetes self-management and education are key components in living a healthy life with this disease. A team approach which includes physicians, nurses, dieticians, and others are necessary to provide care and information on nutrition, medications, exercise and overall management. Access to health care is necessary to assist individuals with the tools they need to control this disease and prevent complications.
Diabetes Success Story
A 24 year-old man with diabetes since childhood arrives at the clinic. He is concerned over his blood sugars being elevated and blurred vision. He reveals he cannot afford his health care visits or his medications (insulin) which are needed for him to live. He can no longer afford the monitoring equipment and supplies to keep his diabetes in check. He meets with the HANDS Clinic team of professionals - Doctors, Nurses, Nutritionists, Diabetes Educators, and ophthalmologists - who provide him with the advice, medications, and supplies he needs to return to feeling healthy. After several visits, he brings his glucose log we created for him into the clinic, and has a lab test done (HBA1c), and his results improve dramatically.
He returns to living a healthy lifestyle, and controls his diabetes with the knowledge and continued care from the Hands Clinic staff.
