Chronic Disease Self-Management Classes

The Chronic Disease Self-Management Program (CDSMP) is a six week curriculum developed by the Stanford University School of Medicine and provided by trained teachers.  Classes are held once a week for two hours.  The classes cover

  • Living with chronic illness
  • Ways to communicate with family, friends, and doctors about your condition
  • How exercise and nutrition impact your condition
